When the time is near for your child to be admitted to the center, you will be further contacted. In the meantime, if you have any questions, please contact the center director at 507-864-7714. If you have not been contacted and your proposed start date is near please contact the center. All parents are encouraged to have back up care if no openings are available. A copy of this form will be sent to you after it is received. Please note that you are placing your name on a waiting list only. This does not constitute the guarantee of a position.
